Hi,
I need some advice- I was approved for SSDI rather quickly following application due to a work injury. I never received the actual DX like so many on here say is stated in their letters. I have multiple issues related to a MVA and I beleive it is either a combination of all or one particular issue. In either case the form is quite daunting but I will do what is asked. There is a section that asks for a witness and one that ask for someone other than medical provider who knows you and your condition I dont trust neighbors enough to ask them to sign as they dont know I am on SSDI and since my accident I pretty much have become a social recluse other than going to doctors\therapy which is quite often. Is it required to have both a witness and to fill in the section asking for someone who knows you to be listed or will it be an issue if I leave those sections blank. Can the witness be a provider? The other part of this is that the WCIC\ my atty is trying to settle the comp case and this requires a MSA - the number that came back was quite a lot and it already reduces my lost wage amount. SSDI will be the only income until\when things improve for me to try and rtw. Based on my luck I will sign sign off on comp and the review will come back to discontinue SSDI benefits and I will not have medical coverage or no money at all. I am currently on medicare. helpful suggestions please |
