Default Is there a form that your psychiatry fills out for disability?

Hi,
I have an attorney but he doesn't know of any forms for my psychiatry to fill out for disability. I had my other doctors fill out the other forms for my physical troubles. Does anybody know of any forms for mental health doctors? Thanks

There is a form.... It's a Mental Health Functional Capacity form. You're doctor should know about it. If not, it can be found on the internet. They need to fill this out for you.

Hello Kimberly,Abbie is correct,but that form like all the other forms for physical trouble i got through my attorney, then doctors would fill them out my disability attorney would go through them before sending out to SSA.This is the first time i heard a disability attorney not knowing of this form,unless his specialty is not on disability.If he isn't get one that is,for your own good!GOOD LUCK this attorney sounds like an ambulance chaser! I just finished going through the same.

Thank you both so much. I'm changing lawyers as soon as I find a new one. He doesn't know what he is doing. The doctor said that it wasn't his responsibility to know what forms he had to fill out and that was up to the attorney to know. Deep sigh. So, thanks for the correct form
