Hi there
I looked thru my SSD stuff and its weird because I have essentially filled out one of these reports except it was specifically from the Disability Determination Service Division and its called Exertion Questionnaire Since Your Disability Began (also I can't find that as a pdf form from SSD or DDS).
ANYWAY- I did have to fill this form out very early in the game. It was before I went to the DDS exam.
:P I have no idea if my reply was to answer your question or if it was a way for me to talk out loud about my SSD scenario. Sorry? Keep us posted about your situation! Lots of luck in the meantime
