X-rays of the lumbar spine were ordered, obtained and personally interpreted by us dated 2/23/11. There is coronal plane deformity. In the sagittal plane there is overall maintenance of lordosis without spondylolisthesis. There is no instability of flexion extension films. There is mild to moderate facet arthrosis worst at L5-S1 and L4-5.
MRI of the lumbar spine dated 8/23/10 was personally interpreted by us. There is mild disc dehydration at L4-5 and L5-S1. There is a minimal disc bulge at L5-S1 with no evidence of recurrent herniation nor neural compression. At L4-5 lateral recess encroaching upon the left traversing L5 nerve root. CT scan of the abdomen dated 9/29/08 was reviewed with bone windows of the lumbar spine. They are within normal limits. There is mild arthrosis of the right greater than left hip with some cystic changes about the anterior superior acetabulum and a kissing cystic lesion about the femoral neck which is mild. There is preservation of cartilage joint space. IMPRESSION: 1. Lumbago. 2. Myofascial pain and paraspinous muscle spasm. 3. Facet arthrosis and spondylosis at L4-5 and L5-S1. 4. Mild right hip arthrosis without cam-type impingement. |
