Default Any Kentucky painies in here?

I am trying to find a pain doc who treated me back in the 90s so I can get my records. I saved them but they ended up being stored in my daughter's garage. Enough said on that. This chap's primary office was in Lexington but also had offices in Central KY. If you think you might be of assistance, please PM me. Thanks.
