There is moderate posterior facet arthropathy of the cervical spine.*C3-C4 there is moderate posterior facet and uncovertebral joint degenerative disease.C4-C5 there is a small central disc osteophyte complex that effaces the ventral thecal sac. There is exuberant bilateral uncovertebral joint hypertrophy and lateral facet arthropathy resulting in moderate right and mild left neural foraminal narrowing.*C5-C6 There is a large left paracentral disc osteophyte. There is moderate bilateral uncovertebral joint and lateral facet arthropathy. These findings result in moderate left and severe right neural foraminal narrowing.*MRI of Lumbar Spine:*L3-L4 there is a small borad-based disc bulge with effacement of the thecal sac. There is a linear area of hyperintense T2 signal within the posterior aspect of the disc indicating a small annular tear.*L4-L5 There is a 12 x 8 mm left foraminal disc protrusion resulting in abutment of the exiting L4 nerve root. The right neural foramen is patent.*
