Default L5-S1 Fusion complications?

Short Story, In March 07 had ACDF with no ill effects. June of 07 had Transacral Disc Fusion L5S1 with no ill effects until 9 months out when symtoms started back. Found out that one screw was broken and another bent!! Also found out that I am not the only one that this has happened to. July of 08 had hardware replaced and was good for 90 days then symtoms came back. After MRI and xrays hardware appear good. recently had 2 back injections so far no relief. Has anyone had similar issues?
