Definitely do a vet check, it could be a number of health problems like diabetes, renal issues etc. but since you've said the box isn't kept clean I'd also focus on that and consider adding a second one if you don't already have 2. The rule of thumb is one box per cat +1. Won't say how many litter boxes we are tripping over in my house.
