Neck - Soft tissue symptoms -
tight, strained, burning ,achy feelings in neck muscles, possibly even shoulder & upper back muscles. If those muscles feel rock hard then they are in spasms. trigger points in those muscles A good PT, DC & MD (if they feel for it) should be able to feel and tell if soft tissues are a factor.. Sometimes MDs focus on the head injury and overlook soft tissue factors. Like with whiplash, soft tissue symptoms can show up awhile after the injury/accident. A friend or family member should be able to help check for trigger points & tight muscles, if those are found then a follow up w/ a good PT is a good idea.

What about pain that spreads along the whole scalp? It wasn't in your list Jomar. Sometimes I can't judge if my brain causes the headache or the neck.

Trigger points (TrPs for short) can cause referred pain - this thread has info & links showing how to locate trigger points..
http://neurotalk.psychcentral.com/thread125577.html I get sore spots on my scalp , it actually gets lumpy in the soft tissue layer, all from soft tissue/ trigger points , sore muscles. I work on the muscles/TrPs and the lumpy sore spots fade away..

Also I think the soft tissue injury maybe the cause of head sensitivity many of us have? Upper neck muscle stiffness also produces tinnitus and hyperacusis
