Ok, did that. It is a triptan - eletriptan. We do have it. I have to make sure they don't make me manic, but I should not think use for just a couple of days could cause that. Use beyond that is not recommended anyway. Might be more of a problem at times when i take Zoloft. Anyway, triptans in general will be part of my discussion with the neuro... so we'll see what falls out, overall.

Gabapentin & migraine summary / update
 
This is my status regarding gabapentin dosing, from starting the med to present, including migraine episodes. not all "twinges" are logged - i had those several days during the initial period.

---------------------------------

Initial titration to 1200 mg / 300 qid:
  • 2012.04.22 - Day 1: 900 mg (300 + 300 + 300)
    ........... "fixed" an ongoing migraine. knockout sedation.
  • 2012.04.23 - Day 2: 750 mg (300 + 150 + 300)
    ........... episodic sedation
    ........... in the am cognitive impairment/slowness, and difficulties understanding fairly rapid speech (isolated incident).
  • 2012.04.24 - 26, Days 3 - 5: 750 mg (300 + 150 + 300)
    ........... episodic moderate sedation initially.
  • 2012.04.27 - Day 6: 900 mg (300 tid)

  • 2012.04.28 - Day 7: 1200 mg (300 qid)
    ........... episodic moderate sedation
    ........... dosage change d/t hormonal migraine risk and
    ........... marked "twinges" alternating temples.
  • 2012.04.29 - 05.16, Days 8 - 25: 1200 mg (300 qid)
    ........... episodic moderate sedation first few days.

    04.29 ... night - migraine(R) mod-severe (pain kept me awake)
    ........... dull occipital pain also.

    04.30 ... am - migraine(R)
    ........... h/a from hell,
    ........... photo- & phonosensitivity from hell
    ........... 3 noxious olfactory hallucinations 20s ea, 3-5mins apart
    ........... occipital pain, tender base of skull,
    ........... bilateral cervical tension (ipsilateral worse),
    ........... bilateral sinus cong (ipsilateral much worse).
    ........... became moderate with sleep.
    ........... later: 30mg codeine + 500mg acetaminophen q4h - 3 doses.
    ........... remission after 24 hrs total, latter 12 medicated w/ partial success.

    05.01 ... am - migraine(L)
    ............mod, unmedicated (used ice, rest, dark etc.)
    ........... later alternating sides, remitted by evening.
  • 2012.05.17, Day 26: 1100 mg (300 + 400 + 400)
    ........... mostly made up missed 2nd qid dose with switch to 400's.

Change in posology to 400mg tid:
  • 2012.5.18 - 5.23, Days 27-32 (1-6): 1200mg (400 q8h)
    ........... episodic heavy to moderate sedation.

    05.20 ... moderate migraine, am L, aft R. hi photo- & phonosensitivity.
    ........... missed am dose
    ........... remedial posology: 400 + 200 + 200 + 400 (first 600mg w/in 2h).
    ........... migraine remission ~2h after 2nd dose.
  • 2012.5.24, Day 33 (7): 1200mg (400 q8h)
    ........... only mild sedation

I expect from here on out I will tolerate the 400 x 3 (tid) schedule ok, in terms of sedation. Haven't decided which is less inconvenient - tid or qid schedule. Cost difference between 300's and 400's is negligible. I will wait a while and decide based on effectiveness if that varies... otherwise six o one half a dozen o the other.

It's way too late again i should go to sleep. I hate hate hate hate how you have to tinker with this thing to make whitespace in the lists. and don't shoot me for the whitespace - i need it, visually.

What I need imminently is to drink more water. had bunches of peanuts. salted. :o now i keep getting thirsty - been through a liter already. :eek::rolleyes: erhm, actually... before i can drink more... i need to visit a certain place! :o:rolleyes:;) thennnn oh yeah, fill up my water bottle, have another mugfulla H2O and thennnnn finalllllyyyyy SLEEEEEEP!!! :rolleyes: yah. ok. yah yah. waves go now fore i forget all this backnforth. kthxbai.

getting migraine... it's building.
 
getting a migraine - left side. this is the highest risk time for me in terms of hormonal migraines, so... later/tomorrow i could be in bad shape. if i disappear for a little while that could be why, depending how bad and how long it lasts. (i don't think the gabapentin is going to touch these ones.)

i need a shower bad but hair was the worst thing, so i went and just washed my hair in the sink. i didn't condition. i am a mess tired and depressed too.

i will do other washing later on assuming this thing doesn't build up tonight. that aspect is unpredictable. sometimes i get a mild one first. sometimes it just hits suddenly.

ghhh. dinner is ready. i am nauseous. ghh.

i slept through my morning dose of gabpentin this morning. i took it around noon (4 hours late). then i meant to take the other two doses 6 hours apart instead of 8 hours but i forgot.

i didn't even remember when i started getting the headache. all i could thnk was i better go do my hair (requires tipping head upside down in sink) before h/a becomes too strong for me to do that. my hair was icko and i didn't want to get stuck in bed w/ h/a for 3 days with icko hair. blech!!! well, as it was i still paid for it, coz h/a gained quota after that tipping biz. :rolleyes: but i do have clean hair now. :p

ok so took 2nd dose after 8 hours, but 600mg to see if it might take out the headache. then iced it ... fell asleep. now - about 3-4 hours later, another 600 mg. it is about back to how it was before washing my hair. i really hope these two close-together doses will kill it dead!

summary for today May 25: gabapentin 1600 mg (400 + 600 + 600)
12:30 - 400 mg
18:00 - migraine(L) onset - mild
20:15 - 600 mg (migraine worse - moderate
23:45 - 600 mg (migraine improved - mild)

00:00 - granny smith. for ?? reason feel it will help??? :confused:

the dealy with my migraines
 
Hi,

i started having hormonal migraines soon after menarche. i used to get one migraine (duration a day or less) once a month. (guess when.) that went on for a long time. i treated them with ibuprofen anwhere from 600 to 800 mg first dose and 2-600mg management doses depending on severity. i must have had an iron stomach because i used to take 800 with my coffee, get ready for work and all i had to do was remember to take more stuff whenever i felt a slight twinge. my work environments were not problematic as far as light and sound.

later on, i started having auras - but not the classical ones. they were not always followed by the headache.

later on again, for a time i had migraines almost every day. that was the first time i medicated with gabapentin - continued for about a year, and then came off and was back to one headache a month.:confused:

at some point the monthly ones started lasting 3 days. Left, Right, then alternating or bilateral. and sometimes i'd get one in between also.

recently --- relatively speaking - last year, they got more frequent again.... bla bla... gabapentin... neuro etc.

here's the deal:

i consider 1 or 2 migraines a month, single day duration, normal for me.

my objectives are:
  1. to use gabapentin or other prophylactic that will get me back to, at most, having only those migraines which i know are hormone related.
  2. to get abortive medication that will treat those reliably and without problematic side-effects, such that neither migraine nor treatment interferes with ability to work. (I can't use NSAIDs any more it seems.)

for now, i'll tell you what!!!! it seems like throwing gobs and gobs of gabapentin at a headache can ... tame it. that's what i've been doing with this migraine... it got better, then worse again.

i'm up coz my sleep schedule is off. headache almost down to nil about 3 am. 4 am ish building up again. i have the codeine stuff but it didn't work that well. instead, i took another 600mg gabapentin. :o:eek:

that's 600mg at 8pm, 12pm, and again at 4 am... 1800mg w/in 8 hours... oddly, it's not sedating me, but i can feel it. and i can feel it in my head and body too. i am not worried if it does knock me out - that will help the migraine too.

only thing is sudden increases make me ravenous. i ate an apple earlier. later a banana but i was still hungry so chased it with a hefty bowl of cereal (muesli style with fruit and nuts... good n' filling). and now i'm at the peanuts but i really have to stop or the salt will dehydrate me. another banana would be better if it gets bad again.

this migraine, at a guess - by the timing, and since it isn't gone... is THE migraine of the month, which is usually the worst of the lot. so just the fact it isn't agonizing right now is very very good. hwoever i think i will now get off computer, get a fresh icepack, and lie down... :o

p.s. in the time i did some edits... i'm feelin kinda woozy but headache is very mild and since it sort of undulates... there are moments where i don't feel it. ahhhh. yes. methinks we gonna have to stay doped up like this tomorrow and next day though...

migraine (L) continues - phono-&photophobia, no nausea today, and no unusual sx so far.

300mg gabapentin q3h mostly works, can be active with care not to bend or move in ways that exacerbates headache. (i sort of have to "float around" lol...) i had to lie down ~6pm with ice, due to significant worsening after cooking. i think chopping parsley really put the icing on the cake (impact). slept some, woke up with headache back to mild.

medication...

09:00 -- 600 mg gabapentin [not 300]
12:00 -- 300 mg gabapentin
15:00 -- 300 mg gabapentin
18:00 -- 300 mg gabapentin
21:00 -- 450 mg gabapentin

Sunday 27 May [added]
00:00 -- 30mg codeine (w/500 useless paracetamol)
03:00 -- 400 mg gabapentin
04:30 -- 30mg codeine (w/500 useless paracetamol)


needed to write it before i forget. hoping i can hold out for 3 hours now, coz there's 3 episodes of the mentalist on, back to back. hehe. hey if i can cook i can watch the mentalist, right? (not necessarily)

not sure how much i'll take before bed... thinking 600 mg. i was hoping to stay within 2400mg today, but it's not been great coverage... getting tired (and sick and tired of it) time to up the ante... other possibility is i might take a dose of the codeine med.

You are doing great keeping track of the doses.
Floating is cool. Deciding how to move . . . gives you a sense of having some control over what is going on.
