Default Wisdom Teeth extracted, some problems :S

Hello everyone, sorry to be a nuisance here but i just had a few questions that may have arisen due to some severe googling and scaring myself.

Okay so i had my wisdom teeth extracted on the 19th Nov, (top and bottom), and all went well as far as i know however, about day 7 i thought i was starting to heal due to the swelling going down etc, but i felt a rush of extreme pain (headache, nausea, etc) that day and an odd taste in my mouth.

So i went to the dentist, and had a quick irrigation and was given a syringe to flush out any debris etc.

However i notice now, that only on the right side, i have a small hard lump (painful if i fiddle with it too much) where the extraction was, and if i press against it a small amount of pus? (yelowish, redish) 'stuff' comes out of the wound (which to me looks like normal gum now, a bit loose maybe), it has a strange taste and it was a taste i had been wondering where it was coming from for the last few days.

I told my dentist and was given more antibiotics to take as i had finished my box of amoxicillin, now im on Clindamycin.

Is there anything to be worried about?
I still have this hard lump on my right side, it's not very big maybe the size of a finger tip.

Thank you, V

Hi vRap,

It sounds like there is some infection going on in that area. It could be localized or it could be in the bone itself. This second round of antibiotics may help, but an xray should be taken when this rx is finished even if your symptoms are gone. Rinsing with warm salt water 3-4 times a day for the next 2 weeks will help to limit the bacteria and keep it clean. Avoid rinsing with any commercial mouthwashes as they contain alcohol and other ingredients that actually irritate the gum tissue and reduce the salivary flow. It is still best to have it rechecked with an xray to make sure the infection has not spread to the bone.

Let us know how you're doing!

thank you for the fast reply Bryanna,
a few things, should i be massaging it to remove the puss (the lump that is), and is savocol for a mouth rinse okay? it is the alcohol free version.
and could the x-ray be a simple one of those small xxrays performed at the dentist itself?

Hi vRap,

I would just rinse with salt water, nothing else. The chemicals in savocol are irritating to the gum tissue and not really going to promote any type of healing. Salt water is the best natural rinse agent that there is.

The problem with massaging that lump is that it could make it feel more sore. The real issue is...... where's the problem exactly.... just under the gumline or in the bone? So yes, a small periapical xray will show if there is a problem going on in the bone.

Thanks again Bryanna, i went to visit the dentist today, and they said the pus like stuff that is coming out does not present the properties of pus, more like a film or saliva type thing, they said that i have a pocketr behind my last molar where the tooth was extracted and that food and other stuff is going into that pocket, and that the lump on the side might be a minor infected but me pushing it is squeezing out the pocket, they do not think it is a bone infection.
So i guess that is good news, she stuck the syringe right into the hole where the pocket was ( i could feel it like really deep maybe touching bone) and she rinsed out and said it was a good clean.

Another question, on my top right where the top wisdom teeth was pulled out, to the outside of the gum i can feel exposed bone, is that normal?

Thanks

Hi vRap,

Ok.... it is important to know that a deep pocket behind that molar is not a good thing because food will keep getting trapped in there and infection will occur. Did they offer you any suggestions on how to manage that area?

Exposed bone is never ok.... are you sure that is what it is? Did you show this area to the dentist?

Hi vRap,

The pocket behind the second molar was actually caused the wisdom tooth sitting so close to that molar. The wisdom tooth caused the bone to deteriorate in between the two teeth resulting in a deep pocket. If you had not removed that wisdom tooth the bone deterioration would have occured anyway and a cavity would have eventually formed on that second molar from food debris getting stuck in there. A cavity can still form in that area if that pocket continues to have food debris trapped in it. What usually works to prevent that from happening at this point is gentle but thorough toothbrushing and flossing in that area. Also, rinsing with xylitol after you brush at night will also help to keep the bacteria down. If the pocket is clean and food does not get trapped for more than a day, the pocket may shrink and tighten up. Keep in mind that plaque is bacteria and anytime it accumulates anyplace on your teeth.......there will be problems.

With regard to the exposed bone........this may or may not heal on it's own. It is something that has to be monitored at your regular checkups and with periodic xrays. If you develop any pain or achiness in that area.... even many months down the road, do not wait to see your dentist.
