Default Anyone deal with FMLA?

Hi All!
Having a lumbar puncture tomarrow. Trying to get FMLA approval for my husband to get off and take me. He works at factory and are on a point system
for days off. 10 points you are gone, he has worked for them for 35 years. Never abused the system, but same rules for everyone. When he picked up the forms from HR, he told my husband he needed a diagnosis and Dr. signature for sure. In looking over the forms, I saw no spot for a diagnosis, and of course you all know I don't formally have one, actually the last time I was there, last week, he said Demylinating disease with unknown etiology.
Anyway, I have had three visits with this Dr.(new neuro) this year, which he filled out and put on the form, and also wrote down I was currently doing physical therapy, which I start Friday. Is that enough to show Serious Medical Condition?? I hate this stuff.
Guess I don't have enough stress, if they don't approve, try to get my DD to take me.
