Default Small fiber neuropathy

I was diagnosed in April with peripheral neuropathy in my feet/legs. My new neurologist says I have small fiber neuropathy in my feet/legs. I also have burning/tingling/electric shocks in my hands. neurologist says I can't have SFN in my feet and hands, that I most likely have carpal tunnel. Going in for a EMG/Nerve test on my hands tomorrow.

First, why the different diagnosis?
Second, Why can't I have both in feet & hands?
Third, as long as the medication works, I guess the diagnosis shouldn't matter.

I am on gabapentin. Right now I take 600mg 3x a day, every three days increase by 300mg until I reach 1200mg 3 times a day. Medication isn't working as of yet, and the side effects are awful
