I just register but read before through some posts and would like to ask for advice. I have constant problems with two root canalled teeth. I have some pressure on them and some kind of neuralgia and paresthesia on that side of my face. Always been told teeth are fine. At first it was four teeth with root canal, other dentist confirmed they are not good and did a surgery. Then he said two wasn't successful and pulled them out and the remaining two said it's fine. I have a bridge there. So long story short I have now that two and think they making me problem. Went to hospital because all the dentist have said everything fine. At hospital they did x-ray and confirmed one is not done correctly but the other one is fine. My wish is to remove them both because I am sick of it. I have them like this since 2009.
Sorry for my English, it's not my first language. I didn't say what teeth are problematic. I have a x-ray and can post it so someone can look it up.

PS the teeth are upper left side I think that is 9 and 10 (first and second on the upper left side)

Welcome Marija! I am in the dental field and can offer you some information here. Your English is fine :)

Thank you for posting the x-rays. The film clearly shows that both of these teeth are infected and that the bacteria has spread to your jawbone. This is typically seen with root canaled teeth because there is no way to sterilize a tooth and make it healthy once it has become infected or once it has been root canaled. Very often a dentist will say that the procedure was not done correctly and needs to be redone or they suggest a surgery called an apicoectomy. Which is what you had done.

Unfortunately, the fact is that how the procedure is done is irrelevant simply because the anatomy of the tooth does not allow access to the hundreds of tiny canals inside of the tooth which means these areas will always contain infected nerve tissue once the tooth has become infected or once the tooth has been root canaled.

You also have posts in both of these teeth which causes further damage to the teeth as well as additional inflammation. Posts are put in to give these fragile teeth some anchorage for the bridge. However, they often cause the teeth to fracture and are a reservoir for the collection of bacteria. It also looks like you have piece of a broken file inside #9 which is not the cause of your problems it is just another source of inflammation.

It is a shame that you have been through so much dentistry in an effort to retain these teeth. The truth is these teeth are irreversibly diseased and will never be healthy again. The longer they remain in your mouth the more widespread the bacteria and inflammation will become. Having the neurological and paresthesia symptoms clearly indicate that the bacteria and inflammation has migrated into areas that will cause you to have systemic/physical health issues related to these teeth.

Replacement options for these teeth as well as the other two are limited due to the deterioration of the bone from the long term infection. Dental implants would be very risky in this area due the infection. Your only option may be a partial denture. You would need to discuss this with your dentist and an oral surgeon.

I know this is scary and upsetting to think about but I urge you not to wait to take care of this infection.

Thank you very much for your reply and explanation I really appreciate it.
I just need now to find a dentist/oral surgeon who is willing to do the extraction. I really hope this will be the answer for many of my mysterious illnesses. I do not mind having partial just to feel healthy again. I am 35 and been having the problems with those teeth since I was teenage.
Do I need some antibiotic treatment, cause no one has told me it is infected?

Hi Marija,

The oral surgeon that removes the teeth may prescribe an antibiotic either just prior to the surgery or at the time of the surgery. It is best to go for a consultation first and discuss everything at that time.

Antibiotics cannot cure the infection that lives inside of the tiny canals inside of the teeth and the medication cannot cure the infection in the jaw bone as long as the teeth are present. The relief that is sometimes achieved from taking antibiotics for an infected tooth is temporary as the medication may temporarily subside some of the inflammation and bacteria, but that effect will be short lived as long as the infected teeth are present.

Oral surgeons will generally not discourage someone from having root canaled teeth extracted. They are very aware of the chronic infection brewing inside of these teeth. It is important for you to understand that dentists are taught to "retain" teeth even when they are infected. That is why root canals are done because they give the opportunity to "retain" a diseased tooth for an undetermined amount of time. It is not a secret that a root canal procedure cannot cure the infection... but this fact is rarely shared with the patient and for many reasons.

You will have to discuss the removable partial with your general dentist prior to the extractions so it can be made in time for the extraction date as it may be necessary to insert it right after the extractions. This is something your general dentist can have done for you at his dental lab after impressions of your mouth are taken. The partial denture would either be delivered to the oral surgeons office or you could pick it up at your dental office when it's ready. Sometimes, the oral surgeon prefers that the general dentist insert it... other times not. Again this needs to be discussed with both dentists.

It is important to disclose your ill health with the oral surgeon and even if you feel that he is doubting the connection to your teeth... don't let that discourage you from expressing why you feel there is a connection.

I went today to dentist. My pain in face is worse so I couldn't wait till Monday. I have been feeling ill for several days which I blamed on possible viral infection. No more waiting. I got antibiotic and dentist agreed one tooth is bad and he is willing to help me. I am having appointment on Monday to discuss everything with him. I told in short what I want and that I want both teeth out. So I hope that I will resolve my problem soon.
Thank you for your help. I found lot of good info here and read a lot how bad root canal teeth are. As a matter of fact several months ago I pulled out one I had. It didn't make me lot of trouble but I decided to get rid of them all. That one was easier but I waited a bit longer for this front ones cause of some financial problems and it looked a bit complicated with that bridge.
I will post next week what I did at the dentist. He is actually oral surgeon which should be better for the extraction.

Glad to hear that you went to the oral surgeon for a consultation. I think you know that it is inevitable that both of these teeth will need to be extracted. Retaining even one of them means keeping the infection as ALL root canaled teeth are chronically infected. So it is important to consider all the issues that are at stake here. I hope you and your dentist are able to mutually agree on what needs to be done so you are rid of the infection and will thoroughly discuss your replacement options before you do anything else.
