The same thing happened to me and i kept on him every single day. I even had my attorney call him several times too. Finally he called me and I told him and all he was worried about is me doubting his excellence. I told him its not even about that its about what I have now..No one is saying your not a great dr just that things happen. Once he realized that I was not out for that he wrote up a new report and sent it in. Keep pressing him don't let it go. I pray that he comes around and document properly for you hun. goodluck and keep us posted sweeite
