Default Signature spot on disability function report?

I got a mailing from social security - a big function report to fill out. I had just got one from the Standard Insurance, with similar questions. (They are the disability insurance co. through my work place).

I filled this thing out, I hope I did a reasonable job. My SSDI attorney specifically advised me not to add additional pages with detail - her point being if I was able to sit and devote many hours to doing a very thorough and detailed report, it might appear I was able to focus and could hold a clerical position with no problem. I hope it's not bad advice. She has stellar ratings on Avvo.

Anyway I see on page 8 there is a spot to fill in your name, address, etc. I see no signature line though. Am I missing something? I thought with something of this gravity, there really ought to be a place to sign & affirm your statement.

I know I can call the social security number on monday or my lawyers office - just thought I would ask my friends here first.
