Default Out-of-Shape or Post Concussion Syndrome

Hello everyone. I suffered from a mild concussion May 12th (4 months ago). During the day I have no signs Post Concussion Syndrome but the other night I tried stretching but became dizzy. How do I know whether this was because I am out-of-shape or not fully healed yet?
