My first pdoc would too. With a clinical presentation of depression and agitation, along with a history of mixed states, these are crucial questions. Regularly asking about these sx monitors the person in a very scrupulous fashion, without assuming the person would take the initiative to report on these. A patient in a difficult state might also forget to report on these. I do hear that it feels frustrating to you. I am sorry about that. :(:hug::hug::Heart:

Then he asked them again. My answer was always no.
I told him I was sleepy, depressed, and agitated (obviously agitated -- physically agitated with lots of movement).
When a person's level of alertness might be compromised - by depression, agitation/anxiety, or sleepiness, they might not always tune in fully, or may give a distracted respons. Repeating the same questions/getting the same answers can help ensure accurate reporting of sx. Or maybe he was having an off day and he was the one to be distracted?

Either way, I certainly appreciate how annoying it can be. In your place, depending how much repetition was going on, I might have had a not-so-nice comeback on the tip of my tongue, like, "I JUST TOLD you that! What are you: DEAF or STUPID?" But my first pdoc used repeat questions sometimes, and he was definitely not deaf, anything but stupid, and very attentive.

He asked me to rank my mood from 1-10. I told him 6 but this was the first time I gave a number to one of these stupid questions because they are USELESS from someone like me who does not think in numbers. I usually decline / refuse to answer questions like that on a number scale.
If it happens again, I might tell him exactly what you have said here.

I was sleepy and physically agitated and tried (but he does not listen well) to tell him about sleep issues.
His response was to announce that he was scripting Trazadone. (And isn't it an SARI?????!!!!!!)
I am sorry that he does not seem like a good listener. I had hoped he would understand the sleep issues.

I am confused about/do not remember the issue with Trazodone/SARI's for you? :o One thing's for sure though: with your current Lamictal grog, I do not see adding a sedating drug, like Trazodone, right now.

He is concerned that I do not have a therapist.
He started to think about ones that might be in my neighborhood but dropped that for now.
GOOD, about his dropping that. Voicing a concern about therapy is professionally indicated, but I sure hope he doesn't breathe down your neck about it.

He said we need more time for the Lamictal and he would see me in two weeks.
Good that he sees you need extra time to adjust to the Lamictal. I like that he wants to keep a close eye on you at this time, but I see that it might be difficult for you if he is not at stone's throw from your home or workplace. How do you feel about it?

The chairs in the room were set up differently so I was sitting close to the pdoc's desk. Then I moved the chair even closer and put my pen and some paper on the desk in case I needed to take notes -- less formal arrangement than last time.
This sounds good.

Hi Mari

Hi Mari, are you going to take the trazadone? I am no doctor, but you don't strike me as someone psychotic. You may be depressed and have anxiety, but that is way different than what this medication is for. I am just concerned about you. I wish you all the best Mari. ginnie:hug:

trazodone indicated for depression/anxiety/insomnia - not psychoses

Dear Ginnie,

I hope it's ok if I butt in here, regarding the trazodone. I think there is a bit of a mixup.

-----------------------
In brief:
1. Mari was not prescribed an antipsychotic. There was no attempt to treat psychosis. The trazodone came up in relation to sleep issues.
2. Antipsychotic drugs are sometimes prescribed to bipolars but not always for psychosis.

-----------------------
TRAZODONE:

Trazodone is an antidepressant drug, and the treatment of depression is its primary indication. It is also indicated (approved) for the treatment of anxiety and for insomnia.

Trazodone has no approved indication for the treatment of psychoses. It is used off-label for a variety of purposes, which might include treatment of psychoses.

-----------------------
ANTIPSYCHOTICS:

Many meds in this class of drugs are also approved for other indications, including mood-stabilization. They are not just given to treat psychotic episodes. So, even when someone is prescribed an antipsychotic, it does not necessarily mean the doctor thinks they are psychotic.

Hope that helps clear things up. :):hug::hug:

Thanks for the explanation I didn't know this either. And with my field it helps me with my helping the adults when I'm helping my parents.

He will have my blood work.

Hi,

I wrote the above on August 4.
Today is August 9.

I had eight doses of 50 mgs Lamictal.
It is renewable two times. The directions on the bottle say to take one pill for two weeks and on the third week start taking two pills a day.
I have 16 pills left.

When do I make the next bump in dose and how much?????

Pdoc did not give me instructions.

He said that we would keep taking the Lamictal.

. . . . some generic b.s. like that.

I have an appointment August 13.

That pdoc is a moron. This is basic stuff ---make sure your patients have enough instructions and enough pills.

Did what I write make sense? Maybe I do have enough pills to last until I see him but the margin of error feels small.

I am confused....are all of the pills 25mg?

how many pills came in the rx? I would take the 50mg for a total of 2 weeks, so 6 more days. I would call to ask what to do then.

high energy at work

Hi,

I slept 3.5 hours last night (Sunday).

At work I was very productive . .in the morning .. getting one task after another done and often doing two or three at a time.

I was laughing (and making others laugh).

. .Then, busy and running from task to task, I helped my friend Max tear down decorations his old colleague had left behind and in a burst of energy spent over an hour taping up his pictures knowing that I was supposed to be in a meeting.

When I got to that meeting, they had made very little progress, so I made them skip around and only deal with the hard parts of the form while we were in the large group. My logic was that we could do the easy parts individually.

I was interrupting. One person stopped me and said something like "I was talking." She was taking tooo long to say what she wanted to say, so I said never mind and stepped outside for a little while.

Then I joked in the hallway to someone that a member in that meeting had "shushed" me.

At the next meeting of the day, I was telling my three colleagues what to do and joking.
We were in laughter much of that meeting.
We encouraged each other.
When the boss checked in on us, we were serious.
When she left we went back to work being silly.
Two of us filled in evaluation forms with smileys and hearts.

I fixed a huge scheduling error that would have been a huge issue for students and a colleague. The boss straightened that out for us . . .
. . . I got the head of IT to set my phone so I could have work emails go to it.

At the end of the day, I asked Work Buddy how he felt about what we had done. He said, "I do not remember what we did, but everything we did we accomplished. " HA!
