Default help with function report

Hi I was wondering how I would go about answering the question do you take care of someone. i take care of my daughter but I have alot of help from my wife and family. I have mental illness. anxiety severe depression panic attacks hep c back pain migraines I am 41 but I feel like I am my psych doctor just said put down yes and explain you have alot of help. I was going to hire a lawyer but my psych doc said you should get it first try. he has my back thanks for your help
