AZ doctors listed above: Dr. Siwek who was seen on T.V. doing Barby's infusion is now the director of that facility, Dr. Turley does them now, HOWEVER, they will only see you for booster infusion if you have had the 5 day inpatient infusion elsewhere, last I talked to them, they were attempting to get set up to do these themselves but they are not at this time.

AZ Dr. Lynch only does LOW dose ketamine infusions, it is a start for someone who has never had ketamine.

There is one other doctor who is setting up to do infusions, he is not ready yet so I will not post his info until he is, his office is getting swarmed with calls and I am not going to add the info until he and his staff are ready.

Thank you everyone for your help and input, I'm trying my best to keep this as updated as possible and could not do it without your help.

Also check out Ketamine Klub on FB. A significant amount of information regarding ketamine doctors has been exchanged lately, Nancy Cotterman Weill (I might have last names reversed) is in the process of attempting to provide both inpatient provider and outpatient provider ketamine doc listings.

Currently I'm on ketamine IM treatment with Dr. Fruitman in NYC. So far results are impressive. They located in Wall street area.

Regarding AZ Dr's.
I called Dr Lynch's office and they told me no they don't do them.

Dr Turley- he is my normal PM. Long story short, in the spring he told me he had nothing else to offer me bu meds. I saw Dr pulley in UT for another set of eyes (I had consulted with him last year before starting with Dr Turley) he recommended ketamine. Dr Turley was ok with it but said he hated to see me travel out of state if not needed. Evidently Dr Siwek ihas opened a new hospital this year called Freedom of Pain Hospital. They are offering ketamine but were not in network for me. I am in UT right now doing outpatient ketamine with Dr Pulley. Dr Turley's office said they would do boosters for me. Never said anything about it being after outpatient. Dr Turley is a great guy, it took a while for him to figure me out (I hide my pain very well and I try not to complain. I finally broke down crying at an appt and listed all the changes I have made
To my life because of this. I just want to be a good mom again. He finally figured me out. He has 6 kiddos just like me. . Anyway, great guy but the communication within the office (messages, appts with psch etc really stink)

I was under dr miler care and he never mentioned any ketamine at all during my time with him,pretty disappointed he was more focus on scs Instead on any other treatments ,my next choice dr Prager but I'm wc patience and he doesn't accept wc patients , bad luck in my case .good luck everyone .jesika

Dr. Joshua Prager does take worker's comp insurance patients. He has been successful in getting wc to pay for some of them. He does a trial of 3 days and then if the patient shows benefit and doesn't have side effects he recommends 10 consecutive days.
Unfortunately most wc insurance will not pay for ketamine.

I got approved by my wc for a visit to an rsd specialist and hope to be able to see dr Prager ,my dream for months I will call his office on Monday and see what they said,I just need a revaluations ,I'm already advance with scs and many lumbar block with no luck but some relief with scs but is pain everywhere and my only hope is get a professional rsd specialist no jut a pmp.
Leslie ,wish you god luck and hope you get all the relief you need,thanks as well for you input ,you gave me hope,for real,thanks and gentle hugs from Jesika .
