While reading article about SSDI, I occasionally find the use of the term, "Disability Report". I know what a Function Report is. I am not sure what a disability report is. Is it the same thing as a function report? Thanks.

List any medical conditions that prevent you from working. So, it's better to not list conditions like adult onset diabetes that is controlled with diet and/or meds, for example. List them from most severe to least severe. It's important that you're receiving ongoing medical treatment for these conditions and are compliant with your treatment plan and taking prescribed meds. Your Alleged Onset Date must be after you stopped working at the SGA level ($1090 per month in 2015). If you have any complicated history tring to choose that date, ask questions. http://www.ssa.gov/forms/ssa-3368.pdf Plan on filling out a copy first to practice on and then when perfected, transfer answers to originals sent by SS. |

Here is what happened and some history. This school year that just finished up in May 2015 was my 25th in a row of teaching elementary PE. On Feb. 28 2015 - I started FMLA due to PN and Mental stress . . . I did not return to work after FMLA began. I resign. Last year I signed a 12 month contract. So, although I did not work in the schools from Feb until now - i will still get a July 20th and an Aug 20th 2015 paycheck based on the 12th month contract I signed last year. Get it? Thanks! |

Totally confused . . . but if this is something I have control over . . . .then why worry about it? Do I have any control over this? Aug 2015 is plenty soon enough for me about benefits!
